1.Social Security Status

Bring one of this items:

a) You should have a Social Security Number

b) If you have never been issued a Social Security Number

c) You must bring in a signed Affidavit (sworn statement) of never having been issued a Social Security Number (NSS-1a).

2.Proof of Date of Birth

Birth Certificate - United States (issued by a state or local government)
Birth Certificate - Foreign (issued by a government entity)
Border Crossing Card
Certificate of Naturalization or Citizenship (N-550, N-560, N-561, and N-570)
Driver License/Learner Permit/Non-Driver ID card with photo issued by another United States state, jurisdiction or possession, or Canadian province or territory (current or expired less than 2 years)
Driver License with photo - Foreign (current or expired no longer than 2 years)
Employment Authorization Card - Valid (I-688B or I-766) with photo accompanied by documentation approved by Department of Homeland Security (e.g., I-797) that specifically states valid United States lawful status as defined by 6 C.F.R. 37.3

3.Proof that you are a New York State Resident

1. Must Be printed (DMV will not accept an e-bill or electronic statements).
2. Have your name and the address where you currently live in New York (the address cannot be a P.O. Box).
